Emergency Medical Consent Notice

Purpose
This notice explains how KairoLink Foundation may collect and use emergency medical information for youth athlete safety. It does not replace any separate medical authorization form that may be required for program participation, events, or travel.
Information Collected
KairoLink may request emergency contacts, allergies, medications, known medical conditions, physician information, insurance details, physical limitations, and other information relevant to athlete safety during program activities.
Use of Medical Information
Medical information is used to plan reasonably safe participation, alert authorized staff or volunteers to known safety needs, respond to emergencies, contact parents or guardians, and communicate with emergency responders when appropriate.
Emergency Response
If a participant appears injured, ill, or in danger during a KairoLink activity, authorized adults may seek emergency medical assistance, contact emergency services, notify parents or guardians, and share relevant medical information with responders.
Parent and Guardian Responsibility
Parents and guardians are responsible for keeping emergency contact and medical information accurate and current. They should notify KairoLink promptly about changes in allergies, medications, injuries, restrictions, or health concerns.
Medication and Treatment
KairoLink generally does not administer medication or provide medical treatment unless specific arrangements, written authorization, and appropriate supervision are in place. Emergency responders or healthcare providers should handle urgent medical care.
Privacy
Medical information is treated as sensitive information and should be accessed only by authorized people who need it for safety, emergency response, program administration, or legal purposes.
Medical Clearance
KairoLink may require medical clearance or restrict participation if there is a reasonable safety concern, incomplete information, recent injury, illness, or activity limitation.
